Enrollment Process and Forms
2017-2018 Student Registration for ALL students
Monday, July 17th – Friday, July 21st 8:00 am – 3:00 pm
Monday, July 24th – Friday, July 28th 8:00 am – 3:00 pm
Tuesday, August 1st 8:00 am – 7:00 pm
Wednesday, August 2nd 8:00 am – 12:00 pm
Registration will be held at the Elementary Campus at 1401 Leaphart Street in West Columbia. This registration is for collection and updating of required forms for the 2017-2018 school year.
Please download and fill out the 2017-2018 Registration Packet and bring with you to registration.
Please be prepared to also bring the following with your forms:
Immunization Record (must be on SC DHEC form)
Proof of Residence: SC Issued Driver’s License AND one of the following: Utility Bill, Cable Bill, Mortgage/Rental Agreement
Emergency Contact Information for Front Desk and School Nurse
Other forms to be completed at registration:
Free/Reduced Lunch forms (Annual Salary and other compensation amounts needed)
Media Release Form
Bus Transportation Form
Home Language Survey (if one is not on file)
